  • If you see that we accept your insurance but still have questions about coverage, please follow the steps below so our team can assist you:

    1. Click the “Contact Us” button on our website.

    2. Send us a message with your insurance question.

    3. Be sure to include your full name, best contact information (phone and email), and the name of your insurance company.

    4. Our team will review your insurance information and contact you to confirm coverage and next steps.

  • Not necessarily. Insurance acceptance does not guarantee coverage. Your plan may include copays, deductibles, or coinsurance that are your responsibility. Our team can help verify benefits, but final coverage determinations are made by your insurance carrier.

  • If we are out-of-network with your plan, you may still choose to self-pay. Some plans offer out-of-network reimbursement, and we can provide a superbill upon request.

  • We offer flexible pricing based on project type and complexity. After an initial conversation, we’ll provide a transparent quote with no hidden costs.

  • Yes. Once we receive your insurance information, our team will review and verify benefits prior to your appointment when possible. We recommend confirming coverage directly with your insurance company for the most accurate information.

  • Please provide:

    • Full name (as listed on your insurance card)

    • Date of birth

    • Insurance company name

    • Member ID number

    • Best phone number and email

    Uploading a copy of your front and back insurance card is also helpful.

  • Some insurance plans require prior authorization for certain services. If needed, our office will assist with the process; however, authorization approval is determined by your insurance carrier.

  • You are responsible for notifying our office of any insurance changes before your appointment. Failure to update insurance information may result in self-pay charges.

  • We accept major credit cards and secure electronic payment methods. Any copays or balances due are expected at the time of service and we require a valid card to remain on file at all times for our clients
