ESA/FMLA Evaluation and Letter
At Release and Renew Mental Wellness, we offer specialized ESA (Emotional Support Animal) letters and FMLA (Family & Medical Leave Act) documentation services for self-pay clients who require official clinical documentation to support workplace, housing, or academic accommodation requests.
These services are delivered with the same level of clinical rigor, ethical standards, and compassionate care that guide all areas of our practice.
What This Service Includes
Each ESA or FMLA request involves a focused, clinically appropriate assessment designed to determine whether the requested documentation is supported by the client's mental health history, current symptoms, and functional needs.
Your service includes:
A thorough clinical interview or evaluation tailored to the specific accommodation requested
Review of relevant records, symptom reports, and functional impairment
Assessment of diagnostic criteria to ensure documentation is clinically justified
Completion of required ESA or FMLA paperwork, forms, or written statements
Clear, professionally written documentation aligned with federal, state, and employer/housing guidelines
One revision or clarification if requested by housing providers, schools, or HR departments (within reason)
All documentation is created to support your needs while maintaining compliance with:
Fair Housing Act (for ESA letters)
Family & Medical Leave Act federal regulations
Ethical and legal standards for mental health documentation
What This Service Does Not Include
To ensure clarity and appropriate expectations, the ESA/FMLA service does not include:
Medication management or prescription services
Diagnostic testing outside of the clinical interview
Laboratory work or lab orders
Ongoing therapy (unless separately established as a treatment service)
Documentation for disability claims, workers’ compensation, or legal proceedings
If additional services are needed, your provider will discuss recommendations.
Processing Timeline
Documentation is completed within:
5–7 business days
after all required information, assessments, and supporting materials have been received.
Expedited services may not be available and cannot be guaranteed due to clinical and regulatory requirements.
Client Suitability Requirements
Because ESA and FMLA documentation must be clinically justified, clients must:
Participate in the required clinical assessment
Provide accurate medical and mental health history
Meet diagnostic and functional criteria for the requested accommodation
Approval is not guaranteed solely based on request; documentation is only provided when clinically appropriate.
